Understanding Support Coordination

Support Coordination is a funded service under the NDIS that helps you:

• Understand your NDIS plan and funding categories

• Find and connect with the right service providers

• Coordinate appointments, therapies, and community supports

• Build your capacity to manage your plan independently over time

Your Support Coordinator acts as your guide, advocate, and connector, making sure all your supports work together smoothly.

Types of Support Coordination

At A1 Coordination, we provide all levels of Support Coordination to meet your needs:

Support Connection

Helps you start using your plan by connecting with providers and understanding how your funding works.

Coordination of Supports

Ongoing assistance to coordinate services, manage changes, and ensure your supports align with your goals.

 Specialist Support Coordination

For participants with complex needs, this higher level of support involves working alongside health professionals, housing providers, and clinical specialists to ensure stability and quality of life.

What Your Support Coordinator Can Help With

Your A1 Coordination Support Coordinator can help you:

• Explore and choose NDIS-registered and unregistered providers

• Schedule and coordinate therapy, support work, transport, and community activities

• Liaise with your healthcare team, support workers, and service providers

• Review your progress and prepare for NDIS plan reviews

• Advocate for your needs and goals during meetings
